Women in active labour – disproportionately Black and/or poor – are denied or delayed urgent care while staff prioritise administrative requirements (forms, insurance, triage bureaucracy) over immediate clinical response, despite clear signs of distress and imminent delivery.
Desired Outcome:
Clear, enforced protocols that guarantee immediate assessment and safe care for anyone presenting in active labour, regardless of paperwork, insurance, race or perceived attitude.
What Could Go Wrong:
Maternal or neonatal death or serious injury from preventable complications; viral videos of neglect erode trust and fuel polarised debates; individual workers are scapegoated while systemic issues remain.
Current Situation:
Documented cases show women in labour being ignored or told to complete forms first; evidence suggests Black women’s reports of pain or complications are more likely to be dismissed.
Action Strategy:
Apply PHC 7-Day Reviews on specific incidents to map timelines and decisions; embed a simple rule of ‘assessment first, admin later’ into policy; publish anonymised dashboards on response times; combine culture change with real enforcement, not just retraining.
