Description:
Failure to complete mitigation actions is preventing the transition from the current risk state to the desired residual state. This transition is evaluated using a 5x5 probability and impact matrix. The review mechanism within the Risk Management process is intended to identify stagnancy and encourage action and risk owners to complete the necessary work.
