Project:
503
Description:
The project faces a risk of mishandling safeguarding disclosures due to the absence of a formal escalation pathway. This could lead to delays or failures in addressing reports of harm, leaving children vulnerable.
Desired Outcome:
Every report triggers a controlled safeguarding response with triage, immediate safety assessment, referral, follow-up and clear accountability.
What Could Go Wrong:
A report is received but no one acts quickly enough, the wrong person handles it, or the child is left exposed to further harm.
Current Situation:
The project has strong intent, but the escalation workflow still needs to be written, tested and assigned to named roles.
Action Strategy:
Create a safeguarding escalation workflow covering report receipt, triage, urgent risk assessment, referral, records, confidentiality and follow-up.
